The Santa Clara County Board of Supervisors, Working Partnerships USA and People Acting in Community Together (PACT) have established the goal that 100% of the children residing in Santa Clara County have access to quality health care through comprehensive health insurance.Application Specialists will help families complete the application form. The family will provide proof of income and county residency. Annual requalification will be required to maintain coverage. Recognizing the variability in employment status, a variety of forms will be accepted as proof of income. For example, families may provide copies of recent pay stubs or tax returns to demonstrate income levels.
